OT: Paging the Undergrounders and Train peeps.

On another place I read, the following has been posted. I offered to
post elsewhere on the offchance a reply might be forthcoming - but not
promised:

Crushed in a door on the underground today
Pardon if I get this jumbled as it all happened so quickly.
I was on my way home at Mile End station, London, waiting for the
Eastbound Central Line train today at 19:28.
It arrived. It wasn't very full. The doors opened, two people got on
before me and I got one foot onto the train but noticed there was
someone else wanted to get on, too, so I motioned for them to proceed
before me. She got on and I began to pull myself the rest of the way in
but the door tried to slam shut onto me, crushing me and banging my
head. As soon as I pulled myself free back onto the platform, the train
whizzed away without a pause. There were some more people behind me
waiting to get on, too, and they were aghast and confused as to why we
had been cut off.
I was shaken by it as I could feel the pain start down the side of my
head and wondered if I had done something wrong. Nobody had taken a long
time to get onto the train. It wasn't packed at all. And normally, there
is a pause between the doors of the train closing and the train pulling
away. This train zoomed away as soon as the door closed.
Is there anyone that works on the Underground able to shed some light on
this? The people standing behind me didn't understand what had happened.
Is it possible for the train to move off when the doors are not properly
closed (i.e., could there have been a possibility of me being dragged
along the platform)? Was it just the wrong place at the wrong time?
